Why don´t you code your buttons instead of using images?
When they are coded they can be fully customized via CSS
A big advantage will be that you display the text on the buttons with a string.
->Translation will be easier
->Templating will be easier
in my opinion
Your idea is good. But I think, many designers will have a problem with that. For example: What if someone likes a shadow or something like that around the text. It is only realizeable as a graphic with a powerfull tool like photoshop.
I will try to allow both in the future. CSS based text and graphical buttons.
At the moment I'am about to redesign the smiley management. Without using database. It will be released with version 0.5.5 and I hope it will be an acceptable solution.

When do you expect 5.5 to be ready?
I think the current approach (which seems to change weekly) is to default buttons to a .css style, but allow an image reference that would override the .css styling.
I've also seen it implemented with a 'hint' attached to the image path browse box giving ideal button image dimensions.

Also, a request. Can you take a look into implementing buttons similar to this http://www.oscaralexander.com/tutorials/how-to-make-sexy-buttons-with-css.html. It's nice to abstract the image from the text label for language compliance and general control. Overall, I think this would be a stronger implementation and the buttons would automatically stretch based on the text label (with padding).
I'd be into making a few image sets if you do.
